Help me study for my Nursing class. I’m stuck and don’t understand.Need 2 SOAP NOTE for different patients. Format to use will be attached with this doc. For patient number 1— Is a 77 y/o male post 14 days COVID f/u visit. This patient was diagnose with Viral Pneumonia 2nd to COVID-19 virus. PMH of : PSVT with ST depression , CAD, Hypothyroidism , Paroxysmal supraventricular Tachycardia, and ASHD. List medications a patient with all these problems would take on a regular basis. Patient is allergic to Sulfa and Penicillin. Patient only c/o residual symptoms on f/u visit like still having difficulty taking a deep breath. All diagnosis must be listed with ICD-code 10. including past diagnosis. Follow the format attached all questions must be answered. For Patient number 2: 68 y/o male came in for f/u visit for his coumadin. His INR is 1.8, PMH A-Fib, Hypertension, HDL, has an internal loop monitor. Same for this patient, list all medications a patient with his condition should be taken, follow SOAP NOTE template, all questions must be answered , all Diagnosis must have ICD-code 10 listed with them. Requirements: as long as it needs to be to answer the question clearly
